Basic Function and Responsibility
To provide ambulatory clinical pharmacy services (disease management and comprehensive medication review) in collaboration with providers in ambulatory sites affiliated with University of Michigan Hospital and Health Centers. This position will provide clinical support to palliative care services (80%) and adult sickle cell services (20%).
Individual must exhibit a professional and positive image when interacting with patients, faculty and staff. Individual must also adhere to a high standard of personal and professional conduct; possess excellent interpersonal and communication skills and problem solving skills; demonstrated ability to interact with a diverse population. Candidates must be able to work effectively with a multidisciplinary team, handling multiple responsibilities in a fast-paced environment.

Supervision is received from the Director Population Health Pharmacy and Community Partnerships.
Supervision Exercised
Functional supervision is exercised over Pharmacy Residents and Students.

Hours/Week: 40 hours, full-time
